Résumé

Overall ischemia of kidney graft is the sum of initial warm ischemia, the cold ischemia during eiter low temperature perfusion preservation or simple cold storage with chilled preservation solutions, and the time required for revascularization of the graft. The ischemic injury increased after reperfusion becomes manifest through immediate up-regulation and surface expression of various adhesion molecules, and activation of circulating leukocytes. The adherent leukocytes release oxygen free redicals and inflammatory mediators. After the initial events of adherence of the circulating cells to the endothelium, various cytokines are further released, and platelets and other factors affect the microcirculation. In this review, the conceptional mechanisms of ischemia/reperfusion injury on transplanted organs are summarized.
