The need for long-term storage of cryopreserved autologous haematopoietic progenitor cells (HPC) in the treatment of multiple myeloma and indications for HPC graft disposal.

Résumé

In treatment of multiple myeloma by autologous haematopoietic progenitor cell (HPC) transplantation a HPC dose sufficient for more than one transplantation is collected, and the remaining cells are stored for use in a second planned transplantation or for salvage therapy if disease relapse occurs. As patients frequently achieve long-term survival after transplantation, long-term storage conditions that maintain the viability and potency of cryopreserved HPCs are required. We studied the use of cryopreserved HPCs from 27 patients for 5 years after collection. Among 210 cryopreserved HPC units, 138 (65.7%) were used within the observation period, 18 (8.6%) were discarded after storage for 2–80 months (median 14 months), and 54 (25.7%) were still stored for future use. There were few reasons for discarding the stored HPC units, with patient death being dominant. Discarding them due to contamination was very rare (0.5%) and doing so because of therapy change occurred in only one case. There were no cases of discard because of low cellularity and/or post-thaw HPC repopulation capacity in this group. This reason for discard verified by the CFU-GM culture test occurred in the year 2022 in HPC units collected from two patients and stored for more than 10 years. These cases highlight the significance of rechecking cell viability and potency before deciding to apply long-term-stored HPC units clinically.
