What can we learn from the Shreveport ammonia fire?

Résumé

An ammonia fire occurred in a cold store. No expert investigation was made and no report was written. By coincidence the author met the refrigeration engineer, a refrigeration expert, who was called to the place after the accident. This engineer explained that the valve which was serviced was installed wrongly. Later, the author met with the fire officer who survived the accident. The fire officer gave the other part of the story with photos and documents from the legal proceedings. The tragedy turned out to be a series of steps in the wrong direction ending up in the accident. It is possible to learn and avoid these faults.
