Summary
A major but little recognized advantage of cryosurgery, compared to other therapeutic modalities for cancer, and a contributing factor to its successful application is that an immunological response may occur. Characterized by local and systemic immunity and associated cytokines, the immunogenicity of the cryolesion, as related to the dynamics of the freezing regime, manner of cell death, i.e. apoptosis vs. necrosis, and balance between pro- and anti-inflammatory cytokines is an evolving paradigm for the treatment of cancer. Comparable to an autoimmune response, the systemic immunity is critical to the destruction of tumour cells beyond the freezing site. This property and the specificity of the initial immune response to destroy affected vs. normal cells, which may leave behind a long-term memory serving to protect the patient from subsequent disease, distinguishes cryosurgery from conventional forms of therapy.
