Summary
Previous work has identified that many of the documented reports of ammonia-related accidents are very inaccurate and that the persistence of these inaccuracies makes it difficult to identify important safety topics when designing new plants or assessing risks on existing facilities. This paper reports the findings of a workshop conducted at the 7th International Conference on the Cold Chain and Sustainability, held in April 2022 on behalf of the International Institute of Refrigeration’s Safety Working Group. The workshop discovered that the errors embedded in government data and propagated by inaccurate press reports promote an undue emphasis on less relevant aspects of safety design while causing more significant aspects to be neglected or completely ignored. The paper addresses significant lessons that ought to have been learned from several case studies and concludes with a review of the discrepancies between what actually happened and what was recorded and reported.
