Summary
In skin cryosurgery, there are 4 important parameters: cooling rate, end temperature, hold time, and thawing rate. These parameters determine the operative effect; however they are difficult to control by conventional methods such as cooling by cotton dunked in liquid nitrogen. Moreover, the relationship between the 4 parameters and cell damage is not yet fully understood. Therefore, both the development of a new cryoprobe which realizes precise heat transfer control and understanding of the lethal temperature for skin tissue are necessary in order to realize more precise and safer skin cryosurgery. It was confirmed that the Peltier cryoprobe, utilized in this study, realized both rapid cooling and precise control of heat transfer through an experiment utilizing a pig skin in vitro. In order to observe the operative effect of the cryoprobe, experiments utilizing guinea pigs in vivo were conducted. The temperature variation with time was measured for various cooling sequences, and we confirmed that the cryoprobe realizes many kinds of cooling sequences and that the cooling sequence has precise repeatability. Stained skin tissues were observed, and the results indicated that the necrotized surface was smooth compared to that of cotton. Also, it was possible for the cryoprobe to necrotize only epidermis and to allow the dermis to survive by setting the end temperature at -20°C. The lethal temperatures of the epidermis and dermis were researched by applying various end temperatures. The lethal temperature of the epidermis was -20°C and that of the dermis was -40°C. The result implies that the Peltier cryoprobe makes it possible to necrotize tissue selectively by controlling the end temperature.
